Afer PRP Feeling Burning, Strong Urge Of Urination

default
Posted on Wed, 3 Jan 2024
Question: Hello, doctor and first, wishing u and your family a very happy holiday. Doctor, I did have the PRP one week ago and the symptoms have increased substantially - burning, strong urge to go every 5 minutes but with an empty bladder. It was 3 injections and very painful. In this extreme discomfort, need your feedback. My sense is that the injections themselves aggravated the pudental vs calm it? Also, should that be the case, do u feel there is any hope of some relief down the road - i.e - such as the nerve calming over time and if u had any suggestions. With the symptoms lit up even more, this is almost unbearable. Thank u as always.

Brief Answer:
I would explain as follows.

Detailed Answer:
Hello,
I have read your new question and understand your concerns.

In your case, some nerve inflammation probably occurred after the PRP injections.

In this case, there may have been some swelling of the nerve itself or adjacent to it.

In my opinion, short-term use of drugs may be of benefit in your case, if there are no contraindications for their use.

I hope this helps you.

Doctor, just one follow up question and thank u for the steroid suggestion. Family doctor also thinks it has merit. My question is about a medication the gyno gave me (Gemtesa) to take once daily which as u know is an anti spasmodic for over active bladder which is not my issue. I am not sure if taking this would be of any benefit or worse, a detriment since there is an urge to go frequently but with no urine since signal is coming from that pudental nerve, not bladder. Did ask family doctor about this med and he was on the fence since he is not a neurologist and suggested I get your opinion. Thanks so much.

Brief Answer:
I think you can try Gemtesa.

Detailed Answer:
Hello again and thanks for the follow-up question.

You can try Gemtesa for the overactive bladder.

The mechanism of action of Gemtesa indeed includes the relaxation of the bladder muscles, but the relaxed muscles may be less sensible towards nerve impulses.

I hope you found the answer helpful.

Let me know if I can assist you further.

Just a quick clarification of saying "relaxed muscles may be less sensible toward nerve impulses" I believe what u are saying is that the more relaxed bladder muscle is not detrimental (although I do not have overactive bladder) and as such, this med would have little to no effect on nerve impulses? Reason I ask is the gyno says that Gemtesa does affect nerves (Beta blocker property) as well as bladder but again, he is no neurologist and just didn't want to take yet another med that may not be beneficial. Thanks, doctor.

Brief Answer:
I would explain as follows.

Detailed Answer:
Hello again and thanks for the follow-up question.

Gemtesa works on the beta-3 receptors located in muscles, the muscles should be less sensitive to nerve impulses.

So, it should improve your urge to empty the bladder by blocking a part of nerve impulses reaching the muscles.

I hope this helps.

Take care.
