Question: 61-year-old male with knife like pain two centimeters to the left Of my vertebral column at the level of my hip with some radiculopathy into lateral upper thigh anterior foot at level of the ankle and some paresthesias in the medial foot Pain occurs only after standing or walking for more than five minutes sitting down immediately relieves the pain MRI revealed L4 L5 foraminal stenosis with some osteophytes and L5 S1 disc bulge of 6 mm impinging on the nerve low-dose prednisone 5 mg per day relieved about 50% of the pain I am concerned I will require surgery can you help me make the right decisionThank you XXXXXXX

I have read your query and here is my advice.

Since the symptoms you are reporting are controlled until now partially by conservative treatment, and the MRI reveals irritated nerve, I think you should go for surgical treatment (decompression of the nerve ).

Prolonged use of corticosteroids also may cause adverse effects, so, this is another factor pro surgery.

Get an opinion from a spine Specialist or Neurosurgeon.
