Hello Doctor, I am XXXX. I was in Amsterdam begining of Nov 2015 and had 3 sexual encounters in 2 days with Commercial Sex Worker. The sex was unprotected Oral sex full fledged (giving and receiving both) and protected Vaginal intercourse. This was the first time I had such encounter and was really guilty for the incident happened. Actually I was in a drunken and heavily smoking during that time.

After coming back I was really stressed and had oral thrush, and mild sore throat. I took antibiotics for 3 days and was ok. After a week i still have mild irritating throat , mild pain on the lymph nodes.
Finally after three weeks of exposure i got tested for following:
1. HIV PCR DNA Qualitative : Result NEGATIVE
2. HIV Combo Test (including p24 Antigeen) : Result NEGATIVE
3. HIV Antibodis - ELISA Test - Result NEGATIVE
Also I had also tested for STD (Syphilis, Herpes) which was also Negative.
I also met a doctor and he suggested antibiotics for a week for throat infection. I used the antibiotics but after a week I still have the mild throat pain (on the left throat and right throat intermittent pain like sqeezing, but it was not continous .
Also I had mild pain under the jaws (tonsiltis).

I was really worried as after using antibiotics why I am still having this irritating throat. Also I developed a rash on the shoulder on 38th day and it has gone automatically in a day.

I met the doctor again and again he gave antibiotics for a week (Cefpodoxime proxetil). Yesterday I completed the course of Antibiotics, Still I have the mild throat, sometime on the left side of the throat and right side of throat or rarely on middle of throat. intermittent squeezing pain. Also one day i had severe head ache on top of my head and gone automatically.

While eating food i dont have any throat pain, but it is irritating, however to clear my doubts I thought to get tested myself again.

On 41st day HIV Elisa Test : Negative
On 44th day HIV PCR -RNA- Quantitative- Negative
On 45th day HIV Combo Test (including P24 Antigen) - Negative.
Please advise on the following:
1. Why I still have the irritating intermittent squeezing throat pain which is mild? I was under antibiotics for 15 days.
2. HIV Test after 3 weeks and 6 weeks are negative (both DNA, RNA, Antibodies , P24 Antigen). can these tests considered conclusive and can forget the episode (We are planning for kids and since 6 weeks i am keeping my wife away and not having sex.)
3. Do i need to get tested for any STD's , i do not have any Symptoms?
4. I was reading the posts on STD, Do i need to get tested for Chlyamdia & Gonnorhea as well?
5. Also while i was taking the HIV tests, I was under antibiotics, will these effect the results.
6.. I am actually a healthy person, the only problem I have is the Gastric problem. I am using the medicines for gastric since 40 days now.
Can I move on with my life normally
Please advise doctor, your earliest reply is highly appreciated.

Thanks

Good evening,

on Sunday night, I had received brief unprotected oral sex (less than 30 seconds) from a woman whom I met earlier that evening. We then had vaginal intercourse, but I used a condom. I did not ejaculate, nor come close. We stopped because there was a significant amount of blood coming from her vagina. I immediately showered and scrubbed the area, but Monday morning I had some burning at the tip of my penis. This has now evolved into steadily uncomfortable sensation around the end of my penis. It is also worth noting that this was at the end of a 3 day drinking binge, and I was fairly intoxicated at the time of the encounter. I had masturbated without lubrication twice earlier that day as well, so I'm wondering if my penis is just sore from that? I went to my doctor this morning, who ordered a Chlamydia test via a urine sample. I, however, have convinced myself that I have herpes and I didn't sleep last night. I discovered what I believe is a canker sore inside my mouth (inside my lower lip), but of course my mind convinced me that this was oral herpes. I cannot focus at work whatsoever, and the more I research this topic online the more I convince myself it's herpes. What are your thoughts? Do you think I have herpes? Is there anything I can do to put my mind at ease?

Hello, on July 19th, I had protected sex with a woman and unprotected oral sex with her as well. then on July 28th, I had protected sex with a woman and unprotected oral sex with her as well. Then on July 30th, I had unprotected oral sex with a different woman.

Then on August 3rd, I got a low fever (99), the following day I got chills, night sweats, and a bad fever (102) that broke that same night. On August 5th, i got a rash that covered my chest, arms and legs (Pictures attached). The rash went away in about 24 hours.

On August 9th I got a full STD screen that included an HIV RNA test and an HIV 4th generation test. Both came back negative.

Then a week later on August 16th, I got another full STD screen again with an HIV RNA test and an HIV 4th generation test. Both came back negative.

I tested the women that I had protected sex with using oraquick and they cam back negative, however, the woman that I had unprotected oral sex with I did not test.

I know the testing window for HIV RNA tests is 9-14 days from exposure, but could this still be HIV?

What are the chances that this could be an HIV infection?

How conclusive is the HIV RNA test? especially since I took it twice?

doctor1 MD

Brief Answer:
HIV Negative.

Between 10/2010 and 1/2011 I visited 5 different female escorts having unprotected oral sex (giving and receiving). A few months latter I noticed a mild burning sensation in my urethra that lasted about a week. This happened three or four times until I finally was able to determined that it happened after washing my penis with a perfumed soap. I once tested it by using the perfumed soap and sure enough I felt the burning sensation for about a week afterwards. I stopped using the soap and everything was fine. Between 9/2012 and 2/2013 I visited four more escorts having unprotected oral sex (giving and receiving). About 30 days after visiting the final escort I started feeling a constant mild burning sensation in my urethra which felt very similar to the soap allergy. This burning sensation lasted about four months until I finally went to my GP and he diagnosed it as Balanitis. At this point I had redness around my foreskin and a small area of crouch itch on my thigh. He prescribed 5 daily tablets of Fluconazole 100 mg. A few days after taking the final tablet the burning seemed to disappear. This was around August of 2013. Since that time I have had several episodes of sensations of mild discomfort or mild burning in my urethra. In December I had a painful crack develop around my foreskin which I treated with Monistat 7 and it cleared right up. I have never observed any drainage from my penis. Should I be concerned that this is a symptom of a STD or do you think that it is something else unrelated to a STD perhaps related to yeast? My GP considers it a yeast related issue and does not consider it a STD.

I will turn 45 next January. In 2010 January, I noticed redness on my foreskin . Around the same time, i also noticed white patches and projections in my inner cheeks. I started developing bad breath (despite brushing my teeth 3-4 times a day) and my tongue also developed minor thrushes (I normally use metal tongue cleaner and have no dental problems). My body is warm whenever the mouth gets a burning sensation and/or when the redness is seen on the foreskin. There is no itching or irritation on the foreskin; but it gets so dry and the skin sticks to the penis head making it difficult to retract unless i use water or other lubricants. I also noticed in the last year, that despite me having a history of pimples on my face, i started getting similar pimples on my neck, head, body, etc. Except for 1, all the others subsided within 2-3 days. Not sure whether this has anything to do with the other irritants. I notice that my urine is light yellowish. I have had no other rashes . I have generally been very healthy till 2010, with no need whatsoever to go to a doctor. I am not promiscuous as a general rule, but i had unprotected oral sex once in 2009 at a massage parlour. The doctors said that there wasn t any STD. They also ruled out HIV. They initially diagnosed it as Balinitis and thereafter Lichen Planus . After it was suspected to be Lichen planus and using some ointment like Advantan and Eliden, it sort of subsided for about 4-5 months when my mouth also was almost normal. From 2010, i had no sex with my wife but after doctors confirmed earlier this year yet again that i have no communicable disease and seeing that the symptoms were not seen, i had unprotected sex with my wife earlier this year for a few months, In the last 2 months, it has started showing all over again and needless to say, I am so worried. I eat spicy foods. I very rarely eat out. I eat lot of meat. I used to consume moderate levels of alcohol - couple of beers or a large shot of whiskey almost daily till about July this year. Needless to say, it is irritating and hopefully i can get some suggestion on further tests or treatment.
