Hi I have been suffering from ear problem and had two mastoidectomies (both cortical mastoidectomies) which could not clear my infection from mastoid. Now doctor has suggested me a modified radical mastoidectomy. I want to know the following: 1. Both the cortical mastoidectomies found my aditus to be blocked. Now the doctor is again suspecting that my aditus is blocked. Will modified radical mastoidectomy clear the problem of "blocked aditus" for ever? 2. Many of my aircells are still there even after 2 cortical mastoidectomies. Will modified radical mastoidectomy clear all my aircells from the temporal bone? 3. I am having a neuralgic pain and doctor says my trigeminal nerve is affected because of this infection. Will modified radical mastoidectomy clear this problem? 4. I am having headache and nerve irritation of trigeminal nerve. Will it go by modified radical mastoidectomy? One doctor also suggested petrosectomy but other doctor denied it saying there is no bone erosion in petrous apex and he is not sure that petrous cells contain infection. Which is better for me - modified radical mastoidectomy or petrosectomy? 5. Does the trigeminal nerve pass through mastoid so that if there is mastoid inflammation that nerve will be affected? 6. When I burp my ear drum bulges out. This is a new thing after 2nd mastoidectomy. When I asked my doctor about it he said this is because normally the air during burping goes to the mastoid but since my aditus is blocked after mastoidectomy so my ear drum is bulging out in this abnormal way. Is this true? 7. I hear a tiiii sound in my ear always. What can cause this - mastoidectomy or remaining infection? Thanks, XXXX.

Brief Answer:
Descriptive answer

Detailed Answer:
Hi,

Thanks for the query.

Modified radical mastoidectomy opens up the aditus permanently as the middle ear, mastoid antrum and air cell system is converted in to single cavity.

Not all but most of the air cells are removed, most means all those...

Dear Doctor,
I am looking for your opinion based on test report attached and history.
History:
2 months back I had cold and cough. on presicription of family physician I had taken medicine for two weeks. But did not get relieve from cold. whitish sticky mucus was continuously coming from my nose and I expereienced hearing loss from my right ear. My left ear already had moderate severe hearing loss since couple of years
I have visit to ENT specialist (1). who further given my some antibiotics and nasal spray (Furamist) and ear drop ofloxacin for left ear having infection. There was no improvement in hearing but there was releif in mucus. But again no improvement hearing.
I have visited another ENT specialist in the last week who has advised Audiometry and impedance Test and also prescribed some antibiotics and Otrivin Nasal drops.
The test report is attached for your reference.
The ENT specialist has advised me for procedure on both the ear. Where as per the report suggest only moderate severe hearing loss.
Please advise should I agree for procedure?
By mistake there are 4 irrelavant pages uploaded along with report. Sorry for the inconvenience.

Hello Dr. XXXXXXX - As per your advise, I am taking the following medicines for treatment of my headache :-

1. Betacap TR 40 (in the morning)
2. Tryptomer 25 mg (at night)

I wish to inform you that I am also suffering from the continuous air-cutting sound (perhaps it is called tinnitus) in my left ear. This problem started six years back only when I got the surgery done for reconstruction of my left eardrum. Of late, I have noticed that the sound volume has increased causing a lot of disturbance which invariably leads to headache. Earlier I forgot to share this information with you. Please advise if this could also be a reason for my continuous headache. If yes, what should I do to get rid of this as it is very disturbing particularly at night. Please help. Regards...XXXX

Hello, I had a Tympanoplasty on 3/7. During surgery I has a CSF gush. Leak lasted 3 days and I was admitted to hospital during that time with extreme dizziness, headache, vision issues. Now 3/26, I am home but still have some dizziness. CT scan done which looks normal per my ENT Doc. Everything I read says my dizziness and headaches should have passed by now. I cannot drive or even walk straight without looking like I have been drinking. Doctor says it will eventually go away. Is there anything additional I should be doing? She did complete the surgery and placed the prosthetic bone in place and explains gush due "probably" to congenital issue.

I had tympanoplasty on my right ear many years ago and I get always infection. I went to ER last week for clogged ear. Doctor put peroxide on my ear for 20 minutes and flush it with water. Sir, I m sending the primary diagonisis. pl suggest whether my daughter can safely treated without sacrificing hearing ability. Also suggest place of treatment. there are quite big perforations in the tympanic membranes of both ear, it is obvious that she has to undergo surgery. The perforation of right ear seems smaller than the left ear and it has involved the membranous part little. So ear saving procedures like tympanoplasty may be done there. As for the left ear I think the matter is worse. Canal wall down procedure (more destructive surgery) may be necessary. Or more ear saving procedures like canal wall up procedures may be done risking the safety mergin.
