Hi,

I have hyperprolactinaemia due to pituitary macroadenoma. My doctor put me on Dostinex 0.5mg course (2 bills per week). After the first two weeks of treatment, I felt of little improvements (decreased gynaecomastia and increased libido and activity). But on the third week, I felt bad again (libido vanished and gynaecomastia and weakness returned).

My next blood test is next week and I'm worried of the prolactin level not being affected. So, I'm wondering if there is anything I can do help lowering the prolactin in blood by, for example, taking more Dostinex or having some other medicine?

Thank you.

doctor1 MD

Hi,

Thanks for the query.

I have perused your attached reports.

First of all, you must be under the care of a trained endocrinologist only.

It is important to have further blood tests for Insulin like Growth Factor – 1 (IGF 1) and 8 am serum cortisol, and also a 1 mg overnight dexamethasone...

Hello. I have hyperprolactinemia. My high PRL was discovered after I had my son cause I never stopped lactating although I stopped breast feeding after two months. It is on the rise since then. It was 110 when was last time tested in May. Since then I had an op to remove my "prolactinoma". It didnt help. I had Bromocriptine, Dostinex, Norprolac. It didnt help. I had 6 MRIs. It was checked also for macroprolactin, my PRL is >98% monomeric (active) prolactin. Last MRI was done in May this year by a new neuro who removed the diagnosis of Prolactinoma. He said my pituitary is fine, there is no tumour. As I said , I do not respond to meds. Where is my PRL produced ? I have seen many different doctors, no one knows...Maybe you know what causes my hyperprolactinemia? I have hypogonadism, irregular or absent menstruation, and as all those doctors I saw described it - hormonal situation completely out of balance.

doctor1 MD

Hello and welcome to HCM,
Prolactin is secreted by the pituitary gland in health.
An increased level of prolactin is seen in tumors of pituitary gland.
Hypogonadism is due to deficiency of sex hormones..
This can be due to deficiency of FSH and LH hormones which are produced by the pituitary...

doctor1 MD

Hello madam, well from the history you have given, you are diagnosed to have hyperprolactinemia, a thyroid profile should be done first, if thyroid is excluded......And if drug induced hyperprolactinemia is excluded....you may require an MRI to look for any micro or macro adenoma. .....well...

symptoms- blurred vision/ recent history of torn retina , left side of face goes numb, left hand numb, tired, irritible at times, history of hyperprolactinemia, weight gain , tremors in my left hand and maybe head that come and go i m 37, recently had a clear mri/ no contrast done and a series of hormone levels tested any ideas?

doctor1 MD

Hi,
Thank you for your query on Healthcare Magic.
The symptoms you have described are suggestive focal brain involvement and with hyperprolactenemia is suggestive of space occupying lesion in the brain.
Contrast MRI brain has to be done to know any demyelination or vascularity .

hi my sister is 22 years old and she was complaining from high prolactin levels and last time the doctor(gynecologist ) asked her to make MRI (MID OF THE SEIAlA TURCICA) and the result came as follows:
a 22 old female patient with hyperprolactinemia of 6 year duration associated with recent visual field defect no previous similar studies for comparison however the current study reveals a well defined focal lesion is noted at the left aspect of pituitary gland measuring 6 x6 mm in its craniocaudal x transverse diameter displaying delayed enhancement compared to the rest of adjacent normal galndour tissue especially noted in the dynamic phase and delayed postcontrast study with mild downward sagging of the sellar floor and minimal focal bulge of the glandular contour still central pitutary stalk with no signifiganct mass effect .
normal optic charisrnas
normal suprasellar and paraseller sturctures
complementary T2 of the brain is unremarkable .
N.B.
Prominant Nasopharyngeal lymphoid tissue.
Right maxillary antrum retention cyst.
Opinion :
the picture has keeping with pitutary microadenoma implicating the left aspect of adenohypopysis displaying size and imaging features as detailed above
so what you think about the report as the doctor said need surgery or there is any other option
