Acute mastoiditis

Questions and answers on "Acute mastoiditis"

Hello, how does my doctor know how bad mastoiditis is without doing a CT scan? My daughter had an ear infection that was diagnosed on March 31 and she was prescribed antibiotic drops. She has a low-grade fever throughout the week which I attributed to the ear infection. Six days later, April 6, I noticed that it was really red behind her ear. We went to the ER and the doctor said that she has mastoiditis and prescribed Amoxicillin 500mg, 3x/day for ten days (my daughter is 5.5 years old). He did not look inside the ear and only looked behind the ear. I have been reading up and it seems that a CT scan is recommended, although when I asked the doctor, he seemed very reluctant and said that he wouldn't do it now, during the treatment, and only after the treatment was finished...and even then, he would have to wait and see. He said that it was caught fast and when I asked how did he know that, he said because I brought her in immediately but it was only by chance I saw the red behind her ear because I have never heard of mastoiditis and never checked behind the ear. I have no idea how many days it was red. Should he being doing a CT scan to check for damage and/or how deep the infection is?

History:
---------
The patient presented with pain in the right side of his head, neck, shoulder, and spine, increasing in intensity for six weeks. He sought care on discovering a 2 cm (0.8 in) abscess behind his right ear the previous day in one of the most painful areas, of typical mastoiditus appearance, of unknown duration since the area was non-visible and never before examined. Was put on antibiotics APO-AMOXI CLAV 875/125 two a day for ten days.

Patient experienced “increase in energy” two days after start of antibiotics and substantial clearing of abscess within four days. However, he then reported a return of pain, and “clunking” sounds of “the bone settling” in the back right of his head. An MRI was performed, summary results provided below. The Patient is currently at home, stable, however reports feeling “not well”. Question: do the results of the MRI below indicate a need for urgent care?

Procedure:
------------
Sagittal T1, axial T1, T2, FLAIR, gradient sequence and diffusion weighted imaging was performed of the brain. Sagittal T1, T2 and STIR as well as axial 2D and 3D MEDIC sequences were performed of the cervical spine.

Findings 1:
------------
The gradient sequence demonstrates a total of 6 small areas of blooming artifact on the axial gradient sequence. The largest is in the grey-white junction at the level of the medial left occipital lobe, with the second largest at the level of the genu of the right internal capsule.

Smaller hypointense areas are identified in the superior left cerebellar hemisphere as well as in the right frontal and right parietal lobes medially.

The findings are in keeping with hemorrhagic axonal injury.

Findings 2:
------------

There are a small number of scattered hyperintense T2 foci within the white matter of both cerebral hemispheres which would be in keeping with either chronic microvascular changes or non—axonal hemorrhagic injury.

There is a moderate amount of increased T2 signal in the pens which would be in keeping with chronic microvascular changes.

Hey, my daughter is in the local children's hospital with an infection in/behind her mastoid muscle. I think they are calling it cellulitis or mastoiditis. She is responding to antibiotics fairly well, and should be released tomorrow, hopefully. They are referring her to an ENT who specializes in a complication to this, something about spinal cord infections. They say it is rare. What is it, what does it do/implications, and what do we look for to keep this from happening again.

Can CPAP use cause chronic mastoiditis? What can you do to treat it if you have to use the CPAP? I am 59 and other wise in very good health. This finding showed up on an MRI that was done because of hearing loss. My doctor did not tell me what to do about it?

doctor1 MD

Thanks for your query.

Yes, Mastoiditis can occur because of Continous Postive Airway Pressure (CPAP).

When the mastoid cells become infected or inflamed, often as a result of an unresolved middle ear infection (otitis media), mastoiditis can develop. In acute mastoiditis, infection may spread...

hi my girl friend has and extremely painful inner and outer ear infection so bad in fact i just got her back to sleep after waking in a crying frenzy i gave her a heated towel to put on it and gave her the med s that she was prescribed and it helped enough to let her fall asleep again.. but as i was consoling her or comforting her i noticed a fairly mid size hard non-visible lump between her right temple and right eye and when applying little presser as i was sliding my fingers across when i noticed she complained of pain from the region but had no knowledge of the lump it self till i broke word s with her about it.. also it is her right ear that is infected.. so i would assume that it could be and probably is a result of the ear being swollen and or infected and not the result of a tumor .. but then again i m not a doctor and this is why i am consulting with you..

Hello,
Welcome to Healthcare magic forum.
The lump you noticed is most probably a palpable lymph node swelling which are commonly seen in case of infections.
These lymph nodes are sites of storage of immune cells i.e white blood cells and they get swollen due to overfunctioning to fight an...

I have suffered with Migrains since I was 16, however I only got one a year, or two at the most. The past year and half they have been more freguent (last 9 months, sometimes 2 or 3 a month). My doctor sent me to have a MRI with contrast. Everything looks great, no tumors, etc, but I have a Left mastiod air cell opacification, and minimal opacification is noted within the right mastoid air cells. I have not had an ear infection since I was in my 20's, no pain in my ears, or behind them, no swelling, no discharge. How could I have this and not have any pain or symptons? I do not have allergies either. Also, could this be the cause for the increased freguency and duration of my migraines in the last 6 months?

doctor1 MD

Hello,
Mastoid is a bone located just behind the ear and its air space is connected with the space of middle ear.So whenever infection occurs in otitis media,it involves air cells of the mastoid.Mastoid air cell disease is usually diagnosed by a CT which reveals opacification in the air cells....
