Hello and welcome to healthcaremagic

Herpes zoster presents clinically as grouped, discrete, fluid filled vesicles associated with pain and burning sensation usually arranged in a stripe (along a dermatome) with a sharp mid-line cut-off.

On the other hand, in case of Insect bite, the lesions might not be strictly localized to just a single dermatome and they may not be strictly follow the sharp mid-line cutoff, as compared to herpes zoster. However, nonetheless it can be really difficult at times to distinguish insect bite from a case of zoster

Herpes requires an oral antiviral e,g acyclovir or famciclovir for a week, whereas insect bite reactions can usually be managed with just topical steroid + antibiotic combination creams.
