Why Is My One Testicle Bigger Than The Other?

My husband is 67 years old and his right testicle has always been bigger than the left and has like two balls in the right one and it has grown over the years but the past several years it seems to be alot larger like the size of a pear . I t doesnt hurt him just concerned it could be something bad

As an Urologist, i understand your concern for your husband.
From your description,on the right side, he has an inguinal hernia.
Above the testis, there's a painless swelling,which may reduce on sleeping,
and become prominent,on standing or coughing.
This's easily diagnosed on examination by any surgeon.
He'll need to be examined by an Urologist, for prostate gland enlargement.
