Why am I having hot sensation on my shoulders, head and a sharp pain under my left lower rib ?

I have been havin hot sensation in shoulders, head and neck area. I have soreness and will occasionl have a sharp pain under left lower rib and shoulders will be sore. I have panic attacks seldom, and blood work, x-rays of chest and ekg come out fine. When i drink caffeine , it seems to worsen and have yawning spells. Could this be too much stress or is something seriously wrong?

It seems to be stress or anxiety manifesting in this way. Last investigation left is ECHO. It would be good if you are able to get it done . it can rule out any heart disease if its there.
Then we can focus on Relaxation therapies which you can do now also.
Do Yoga , Exercise Avoid tea coffee smoking or caffeine.
