General & Family Physician 's  Response
Hi,

In your case 686IU//ml is considered to be high. Start treatment right now. You should also measure periodically SGPT, SGOT levels, USG liver or if possible fibroscan of liver also. Treatment must be lifelong per se viral load must be kept under observation periodically to avoid further liver damage. Consult hepatologist for better management.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Pragnesh Limbachiya,
