There's a lot of things. Ex: weak heart valve, poor diet, predisposition, stress. This is going to sound kind of crazy: But were you watching What Not to Wear tonight? Is this what inspired the question? (They had a girl on, who was an ambassador for the Red campaign. She was active, and nineteen when she had a heart attack.)
