What Is The Suggested Treatment For Osteoporosis?

I am 75-years-old female, 5'4", suffering from diffused osteoporosis. Doctors prescribed me calcium supplements. which i have been taking for the past five years. But lately i did an X-ray exam which revealed diffused osteoporosis. And my condition is worsening. My family physician says that my T11 vertebrae is compressed and reducing in size. Till now no one has suggested hormone replacement therapy. Is it a possible course of action?

Geriatrics Specialist 's  Response
Hi,
Going through you history, I think you should be treated for osteoporosis. You have already T11 compressed vertebrae and other complications could also arise due to fall.
The suggested treatment is as follows:
Non-pharmacologic therapy
1. Physical activity with resistance training to increase bone and muscle strength and it also reduces the risk of falling.
2. Participation in outdoor activities in safe manner to make you more active
3. Program of walking, sitting and standing exercises along with low impact or water aerobics.

Pharmacologic therapy:
1. Hormones therapy with conjugated estrogen for indefeinite period.
2. Another optional one which I would also recommend is selective estrogen receptor modulaators like Raloxifene. (mostly indicated with vertebral fracture)
3. Along with calcium, additional Vit D therapy

Precautions: Avoid falls, make surroundings elder friendly.

Any other queries, happy to help you again.

Orthopaedic Surgeon, Joint Replacement Dr. Ravi Nayak's  Response
If I was your doctor, in addition to calcium supplements, I would also prescribe vitamin D supplements and tablets known as bisphosphonates. The older regimen of hormone replacement is not preferred, however a new hormone called teriparatide can be considered in your case.
