Hello,
The most common causes of a swollen earlobe would include an infection of the earlobe due to an inflamed piercing.
As you do not have any history of ear lobe piercing, it is most probably due to either bite by any insects or infection from ear.
The tissues in the ear lobe are soft and very loose.
Thus it is usual to have the inflammation and infection spread in short time.
As your doctor advised, please continue the antibiotics.
If you have the pain, please take pain killers like tylenol or ibuprofen.
if the pain and swelling does not decrease even after the course of antibiotic, it is better to choose either alternate antibiotic or minimal incision and drainage.

Hope this helps.
