What Is The Cause Of Positive Pregnancy Tests Despite Negative Ultrasound Tests?

Hi, I am having some issues with my OBGYN. We have had 15+ positive pregnancy tests both urine and blood since 1/7/14. Last Monday my hcg levels were at 4628 and Friday were at 36892. In the last week we have been sent home to await the bleeding of a miscarriage, when we had no issues with that we were told it was ectopic then that was ruled out, we were told it was completely healthy and lastly (yesterday) that we should be expecting twins. We have had 2 trans vaginal ultrasounds, one of which showed nothing and the other which showed a thickened endometrius. I am having no pains out of the ordinary, no bleeding, and am stressed to the max... what is going on? What should I do?
