What Is Equiv Dosage For Hydromorphone ER And Rescue Hydromorphone?

hospice nurse: patient receiving 40 mg oxymorphone bid (80mg/24hrs) and Hydromorphone 12mg/24. pain neuropathic. spinal cord compression. Want to suggest switching to same drug for long acting & rescue. What is equiv dosage for Hydromorphone ER and rescue hydromorphone. What about use of Exalgo for ER?

Alternatively, Methadone is useful for complex pain syndromes and neuropathic pain. It generally is reserved for cases in which first-line opioids (oxymorphone, hydromorphone) are ineffective.

Exalgo is an extended release formulation of hydromorphone hydrochloride.

Hydromorphone ideally should be used with patient-controlled analgesia (PCA) by a microprocessor-controlled infusion device with a lockout period after each demand dose, to prevent overdosing.
