What Does This Thyroid Test Report Indicate?

Hi , I was hypothyroid, and took Levothyroxine 50 Cg, On July TSH = 0.45. My doctor told take one more monthy . Then Sept , level TSH= 0.006. Doctor told stop Levothyroxine and Oct, lab test again as below . I am hyperthyrod now. and Recommend starting methimazole 10mg twice a day x 2 weeks then 10mg once a day. Should I take medication or not? Why I from hypothysoid to Hyperthyroid just one month. Is TSI = 6.8 grave disease? Thanks you for your help.
