Hi welcome to health care magic...

Since how long do you have diabetes????

You are having high creatinin and urea level with pottasium level also high...
This arise suspician toward kidney damage like nephropathy....

So USG first of all done along with urine examination also....

If needed kidney biopsy taken and sent for histopathology examination for rule out nephropathy...

According to report than specific treatment guided

If needed IVP can be done

Sugar level should be controlled with medication according to need...

Take care

Consult physician for examination and further work up
