i have one pt who is suffering for acute Rhinosinusitis and the doctor was recommended that he will attend a sergury, at the time of investigation he also suffering from chest pain which the result become polmunary embolism. and the result of ECG is inferior myocardial damage. so what do you thing this case will be and how it can be treated?
Thanks for your question on HCM. In my opinion you should immediately consult cardiologist for this. Pulmonary embolism with inferior wall ischaemia suggest right sided heart damage. And we need to treat it promptly as it will cause hypotension and bradycardia. We first need to identify the extent of emboli. So get done CT PULMONARY ANGIOGRAPHY. And start fibrinolytic therapy to dissolve emboli. Once patient is hemodynamically stable then plan coronary angiography to rule out coronary obstruction. And if needed stenting should be done. So you need cardiologists and cardiac intensive care set up to treat such patients.
