What Does The Following Test Result Indicate?

Bladder Adequately distended, smooth mucosal outline, no stones, no mass Uterus Slightly bulky sized for age measuring 64*52mm with a posterior wall myoma of 22*20mm, empty cavity and normal secretory endometrial thickness of 15mm. Ovaries Normal sized right ovary but left ovary is enlarged containing a cyst of 64*47mm with a thick septum, a solid mass and a mesh of fibrinous stand. Adnexae/POD Free, no mass, no fluid collection. Please explain : can a surgery take place with this kind of result, and what should be the next step?
