What Does The PSA Test Result Of A 54 Year Old Male Indicate?

I m confused....54 year old male. Routine PSA test in 2015 was 2.94 Routine PSA several months ago was 13.7 Month later had 12.6 PSA and 7% free PSA Two digital exams both came back nodular 12 core biopsy with results yesterday....all came back clear = NO CANCER Between the high PSA, velocity, low free percent, two digital exams and low free PSA....can this biopsy be correct? Doctor is having me take Flowmax for 6 weeks because I have what he considers significant retention ie: 220ml.... Is urinary retention enough to cause the high PSA or is it more likely there is a transition zone cancer that wasn t part of the peripheral zone biopsy? I love the no cancer diagnosis, but it doesn t explain all the other stuff that would indicate an aggressive prostate cancer. Appreciate your thoughts on my doc s next steps. 6 weeks of flowmax, PSA and then do a flow test in an office visit at the end of the 6 weeks.

* As per my surgical experience, the PSA result of total amount is not of significance for prostate cancer as prostate cancer is only signified via free PSA results which are low in your case.

* The total PSA may vary from time to time and even digital rectal examination, prostatitis may raise the total PSA value, but not that of free PSA unless there is prostate cancer.

Hence your biopsy result is 100 % true and you do not have prostate cancer, again confirmed on clinical grounds by smooth prostate evaluation.
