What Does Sharp Pain In The Lower Back Indicate?

I have a very sharp, well-defined pain in my lower back that is persistent (hasn t gone away for two days), pain in both hips, discomfort in the left side of my abdomen, and diarrhea alternating with constipation. I don t have a fever but I am also unusually fatigued. I have an appointment with a gastroenterologist next week. Based on these symptoms, is it risky to wait one week for that appointment or should I go to an urgent care or ER?
