What Does Severe Dizziness While Suffering From Diabetes And Hypertension Indicate?

I am a 61 yr old male with diabetes, hypertension, and am morbidly obese. I am experiencing extreme dizziness while even while sitting still and when walking. It feels more like I am spinning rather than the room spinning. There are sensations of nausea with this. I went to the ER yesterday. They took a head CT scan and found nothing. They referred me to ear, nose and throat specialist for treatment. I am still concerned that this may be something neurological rather than vertigo. The sensation can be quite alarming. I have a few things that I have wrong with me that may be sources for this. I have 2 herniated discs in my neck at C-2 & 3 or 3 and 4. The xray taken last year showed slight compression on the canal with the spinal fluid and spinal cord. The compression bulged only slightly into the canal. My neck has been painful over the last 2 weeks. I also have an umbilical hernia which protrudes out of the belly onto the surface about 2 to 3 inches. I have been taking opiates for pain for about three years, and so I fight possible constipation with laxatives such as Swiss Kriss, Docusate. AlsoI take about 325 mg of magnesium several times a week. The reason I mention the bowel issue, is because it feels like the dizziness and nausea get worse if there is pressure in the bowel that comes before a movement or with gas. I’ve been experiencing the dizzy sensations for almost a year now. Much less pronounced than they are now. Over about the last week is when they have gotten very severe to the point where it is difficult to function normally. I have not fallen or passed out About 2 months ago I had a series of heart monitoring tests including a Holter monitor for 7 days and an event recorder for 30 days, as well as a heart sonogram to see if this issue is heart related. This has been ruled out. Can you shed any light on this please?
