What Does Prominent Annular Fissure Means In MRI Results Of Disc?

I had back surgery months ago, for bulging disc and pain in my right leg, disc was hitting nerve. Now the pain is 100 times worse, recent MRI done shows disck protrusion slightly displacing S1 nerve root in the lateral recess and extending into the proximal aspect of the foramina with effacemen. The L4-5 there is a slightly more prominent annular fissure in the right foraminal zone associated with a shallow protrusion. What does this mean?

An annular fissure is a tear in the annulus fibrosus component of the disc. Most are asymptomatic, however some are painful. The defect allows ingrowth of nerve endings and granulation tissue. Fissures near the dorsal root ganglion are especially likely to be painful. This is causing the pain in your back.
