What Does Chest Pain While On Treatment For Lung Cancer Indicate?

My brother is taking opdivo treatments for his lung cancer and says he is having chest pains. He also still smokes and I know that he has a smokers cough that is awful. Could he be having chest pains from the smoking or is the chest pain a side effect related to the treatment?
