What Does Brucella -ve , Blood Culture -ve,EBV EBNA +VE ,VC IGg +VE, IGM VC -VE, LDH 503 Indicate?

an 8 years old girl complaining of high grade fever since 3 months associated with arthralgia, sore throat cholestatic juandice
on examination hepatosplenomegaly , mitral rugurge
investigations shows mild hypochromic anemia with thrombocytosis, ALT 66 AST 62 PA 80%
ESR 110/120 CRP +VE Autoimmmune markers -ve bilirubin 2.6, Bone marrow aspiration normal, typhoid and brucella -ve , blood culture -ve,EBV EBNA +VE ,VC IGg +VE, IGM VC -VE, LDH 503

Hello and welcome to HCM,
The symptoms of fever, arthralgia, sore throat and cholestatic jaundice suggest viral infection.
Associated hepatosplenomegaly along with jaundice shows that there is involvement of reticulo-endothelial system by the infectious agent.
Investigations show positivity for EBNA which is an Epstein Barr virus antigen.
The Ig G is also positive for Varicella virus.
Thus, overall there is current infection with Epstein Barr Virus and past infection with Varicella virus.
Raised levels of AST and ALT are due to involvement of liver parenchyma.
Raised ESR and positive CRP suggests an underlying inflammatory condition.
Positivity for EBNA is the probable cause of raised ESR and CRP.
You need to consult your treating physician for further treatment of EBV infection and thus symptoms.
