Hi,

Usually the heart patients or patients who had stroke previously, are on blood thinner, hence they are having more chances of bleeding from rupture of a blood vessel. This bleeding may be internal, which is not visible outside the body. However, many a times, this blood from the ruptured vessel, will come out of the body through natural orifice of body like nose, mouth, anus or urethra etc.

This phenomenon as well as chances of bleeding is same, whether the patient is conscious, unconscious or comatose. However, other causes of bleeding like hypertension, diabetes or bleeding disorders, should also be looked for and treatment done accordingly.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
