What Does An Increase In PSA Level Indicate?

I am 65 and saw my PSA score go from 5.5 to 6.2 in 3 months. I have no symptoms and the digital exam was reported as normal. My PSA did spike to over 12 about 10 years ago and then declined to under 4 on a retest a couple months later. It s been 3.8 and 4.2 the last 10 years. Should I seek a biopsy if I see an increased or same score at my next appointment? and the digital exam is still normal?
