What Does A Swollen Lymph Node Below The Right Ear Indicate?

Yes I have a swollen gland directly below my right ear . I have had this before with severe itching behind the ear. The swelling is large and this comes and goes in the same spot . Swelling varies. What is this and which compress is best hot or cold?
