What do swelling and a lump in the neck indicate?

I have a swollen neck, lump and having problems swallowing MY primary set up an appt. with ENT. I also have graves disease which was treated by radiation and I also have MS and celiac disease. Do you think this is my thyroid possible nodule or an ENT problem?
