sometimes, usually at night, I get what seems like a spasm in and around the rectum area. The instinct is to go poop but it is never achieved. It is very painful in a dull way. It leads to profuse sweating, and even momentarily losing consciousness. It comes and then it goes in about a half hour or less depending on severity,

The painful spasm you feel around the rectum area suddenly at night is called Proctalgia fugax. Usually, no known cause is clear. However, it can be very painful as in your case.

I recommend consulting with proctologist first and run some primary diagnostic tests like colonoscopy, digital examination, and others to rule out other more severe causes for proper treatment.
