What Do Irregular Bowels, Low Appetite, Nausea And Pain Under Ribs Suggest?

I am an Air Force active duty member. The medical professionals do not look into problems, they throw pain pills at them. I am a 23 y/o male. For more than two months now, I have been eating very little, sometimes because I don t feel hungry, but some days I eat a normal amount. My bowel movements have been so irregular, there is no pattern, no consistency, no pattern. Most days I do have at least one bowel movement, though. I feel tired and dizzy at least half of every day. I feel nauseous at least half of every day. I have pain mainly in my left side just at the rib line, sometimes very painful, but usually a dull ache. I also have random pain at my shoulder blade or just below it. I can go all day and feel perfect, then feel terrible and in pain. No pattern with eating that I can tell, just completely random. However, I have noticed the symptoms Start almost always at night. Had an ultrasound done, but I felt perfect the day of it. They found nothing. I m sick of being in pain and being nauseated and then the next hour feeling perfect. It makes it impossible to visit the ER. I also have been less frequently in my right side upper back at or under shoulder blade. Twice now I have almost called 911 because my chest tightened up and I felt I couldn t breathe and became really dizzy. That only lasts for maybe around 20 minutes. Don t know if that is related? No position, movement, or meals such as high in fat seem to make a difference on the timing or the pain level or frequency. Urination is fine, stool is not pale color, just very irregular. Any advice, or maybe something I can suggest to my doc to try? I am so sick of feeling like this

General & Family Physician 's  Response
Hello dear
I appreciate your concern
Tightness in chest and not able to breathe at your age might be related to heart or lung disease like vasospatic angina or acute exacerbation of bronchial asthma
so please ask your doctor for EKG and blood investigation for serum cardiac markers , CBC , AEC and allergy test

Your gastrointestinal symptoms are suggestive of irritable bowel syndrome, liver disease like fatty liver or acid peptic disease

you should consult a gastroenterologist for battery of tests like liver and thyroid function test and an endoscopy if need be.

Another possibility is stress and anxiety disorders
Avoid fatty fried spicy and junk food
say no to smoking and alcohol
Have fresh fruits and steamed vegetables
Have plenty of dietary fibers

Hope this answers your question
Best wishes
