What Could Heart Burns During GERD Infection Suggest?

I was diagnosed with GERD six years ago. I had an endoscope at the time. I was put on Nexium which worked and I take it periodically to treat minor flare ups. For the last three weeks I have had terrible heartburn and the Nexium does not seem to be working. I went to the doctor who thought I had a virus. He did an ECG just to make sure my heart was fine. The heartburn has decreased somewhat but it is still very bothersome. I have no other symptoms and had one day where I had diaherra about a week and a half ago. I have no loss in appetite. The heartburn seems to be clearing and then comes back. I am very discouraged and just want this to go away. Could a stomach virus cause a GERD flare up like this? I am only on 20mg of Nexium. Should I be doubling my dose?

GERD is gastroesophageal reflux disease. It indicates that there is reflux of stomach contents in esophagus causing you the symptoms of varying heartburn. Hence I would like to advise you the following in such a situation:
-Get an upper GI endoscopy.
-Continue Nexium same dose.
-Take an antacid on empty stomach.
-Small frequent feeds of soft bland diet.
-Control of stress and anxiety play a major role in such a situation.
-Further management as per the opinion of a gastroenterologist. If needed you will need further investigation and management as per the findings.
