Orthopaedic Surgeon 's  Response
Hi,
this appears to be a clear cut case of hyperuricemia (not necessary gouty arthritis ) Hyper uricemia beyond a cerain limit only is referred to as gout.
In this particular thing the most commonly affected joint is the metatarso- phallangeal joint of the great toe.THis is typical. In acute conditions ( hot phase ) febuxostat has no role to play. The only treatment is cold packs and NSAIDS.It is only after the inflammatory signs have disappeared that u can start uricosuric drugs.
