What Causes Vaginal Pain And Bleeding After Intercourse?

I am a 45 year old woman. My husband and I had pretty aggressive sex Friday and Saturday (totally consensual). I got up to pee this morn around 7 am and was unable to. It felt blocked but the sensation that I felt was crazy too. It was a cross between an extremely intense orgasm mixed with the urge to pee immediately. Quite painful to say the least!! I kept trying every few minutes as that urge and pain never fully subsided. It dulled some but I had the I have to pee feeling. Like 30 min later I tried again but this time I spread my inner thighs trying to spread my vagina open and I bared down. When I peed it was aprox a teaspoon n pure blood. It also had a small thin clot in it there wasn t any urine. And it was as if I could feel that small clot come thru the uretha as that was the height of the pain. Since it s happened another 7 times. There was just one that yielded another aprox teaspoon tho. The rest were 1/4 tsp at best. That constant pee feeling stopped around noon and the more intense sensation will return only upon urination. Can you tell me if I did this from my last two days? Or is that just irrelevant? Please help. What should I do?

Hi there,

It sounds as though you might have a urinary tract infection. Rough sexual intercourse can make you more likely to develop it, because the bacteria from the outside skin can get pushed into the bladder. This will be especially true if you did not go to pee afterwards to flush whatever bacteria out.

There can be blood sometimes yes, and it can appear really dark if you haven't been drinking enough water. If the blood has been sitting there for a while, then yes, it can clot up as well. That constant urge is due to the irritation from the blood and the infection, and usually is just an urge but there is not much urine left in the bladder.

However, several teaspoons of urine through the day is very little, and I am worried that you might be retaining the urine. How much fluids have you been drinking? If you have been drinking a lot of water and you are still not peeing a decent amount, I would advise you to go to your local doctor's just to check if your bladder is actually full. Sometimes, the bladder gets overstretched and a catheter is needed just to drain the bladder once to help it function again normally.

If you have actually been holding back on the fluids, or if there has been many episodes of tiny amounts of urine that could make the output total up to a decent amount, then it is more likely that it is just irritation of the bladder. In that case, a course of antibiotics like ciprofloxacin or amoxicillin will deal with the problem easily. A urinary alkaliniser like potassium citrate or Ural will also help with the urge and speed up recovery.

Don't be afraid to drink enough water, at least two to three litres a day if you have no other health issues. Producing enough urine dilutes the blood and infection, which makes the irritation and urge less. It also helps to flush out the infection and makes you recover faster. You should feel much better in a day or two, if you haven't improved by the third day, seek help with your local doctor's to possibly change the antibiotics or even send the urine for a culture test.
