What Causes Tightness, Tingling And Numbness In Toes In A Diabetic?

I am an insulin defendant diabetic and take gabapentin for neuropathy.Lately it doesn t seem to be working as well and I am taking 3 pills daily. I now not only have tingling and numbness but the balls and toes feel tight as though enclosed in clay and are somewhat painful. Tried ice, tried heat but nothing helps. What can be wrong?

just maintain sugar level.
