General Surgeon 's  Response
Hi.
Thanks for your query.
Noted the history about your wife and understood your concerns.
Female patients get tight stool and fissure-in-ano after delivery; this is quite common but most of them recover well after proper management and do not get the symptoms unless there is hard stool or any such problems.
As per the history you have provided about your wife, she is suffering from constipation, hard stool not allowing fissure-in-ano to be healed completely; in fact is getting recurrence as the instigating factors are still present.
I would advise her the following in such a situation:
Continue Cremaffin or cremaffin plus preferably in a dosages that gives loose stools or change over to another ones that gives soft and smooth motion.
Sitz bath in a tub with warm water before and after passing stool and in the evening.
Get examined by a General Surgeon so that a proper evaluation of the problem is done.
If the medical line of treatment fails to give a cure she will need Lord's anal dilatation or lateral cutaneous Sphincterotomy to get a permanent relief.
I find this answer helpful
