What Causes Spotting While On Birth Control Pills?

Hi I started the pill 4 years ago, last month on day 11 of the pill I began spotting which continued through to the end of the pill pack when I began the withdrawal bleed. During the withdrawal bleed my vagina became uncomfortable along with an occasional urge to pee when I didn’t need to, but no pain or cramps. It was also too uncomfortable to use tampons. I finished the withdrawal bleed yesterday and started my next pill pack. Today I still have vagina discomfort, slight burning, bladder pressure to pee but it’s not bad. Also when I pee and wipe, my tissue is slightly blood stained but nothing major. I’ve had a clear pelvic ultrasound. I’m almost 25, what could this be?

Hello,

Your symptoms can be due to subclinical urinary tract infection or vaginitis.

I will suggest you to get gynecology checkup done including per speculum and per vaginal examination. Thai will help to rule out cervical erosion or polyp as a cause of your spotting. Please get urine routine examination and culture and sensitivity test done.

A course of a broad spectrum antibiotic containing Amoxicillin and Clavulinic acid two times a day for 5 days and local vaginal pessary containing Clindamycin and Clotrimazole once at night for 7 days will be of help. Please seek prescription support from your treating doctor for the same.

You can take tablet containing Tranexamic acid and Mefenamic acid combination as needed for controlling spotting. If needed add tablet Regestrone once at night along with oral contraceptive pills to control the spotting.
