What Causes Sleepiness And Wheezing?

I TAKE NO MEDS EXCEPT ZYRTEC BECAUSE OF A DOG ALLERGY. TIRED FOR QUITE A WHILE. HAD SEVERAL TICK BITES LAST SUMMER AND AGAIN BEGINNING IN SPRING. STRANGE SENSATION LIKE A SHOCK IN MY HEAD. EXPERIENCE SUDDEN EXTREME SLEEPINESS AT TIMES PARTICULARLY WHEN DRIVING. HAVE DEVELOPED WHEEZING HUSBAND SAYS WHEN THIS HAPPENS HE CAN NOT SLEEP, HAD CHEST XRAY SHOWED NOTHING NO FLUID BUILD UP OR TUMORS. LAST ROUTINE BLOOD WORK WAS GOOD. MY DR. CAN FIND NO REASON FOR EITHER THE TIRED FEELING SHORTNESS OF BREATH, WHEEZING. BLOOD PRESSURE ALWAYS GOOD RUNS ABOUT 110 OVER 68 PULSE RATE 62 WEIGHT 189 LBS.

Hello,

Thank you for asking at HCM.

I went through your history and would like to make suggestions for you as follows:

1. First of all, I would like to know more about all your symptoms since very beginning. i.e., nose symptoms, cough, wheezing, their frequency, day/night variations, seasonal variations, any skin symptoms, stomach symptoms, headache, whether you have dog, how dog allergy was diagnosed, etc.

This will help me to understand your complaints better and also to make suggestions for you more specifically.

2. As you have allergies and developed wheezing/breathing difficulty, with X-rays and other reports normal, I would first think of possibility of asthma.
I usually suggest my such patients Spirometry with post-bronchodilator reversibility testing which helps to measure your lung capacity as well as type of your lung disorder, if any.

3. I would also suggest you allergy testing for common air-borne allergens like house dust mites, molds. pollens, insect proteins, animal dander, etc. This will help you identify the cause of your allergies as well as to know the measures to avoid them.

4. Based on allergy testing, an Allergist-Immunologist may prescribe you allergen specific immunotherapy which works on immune system to gradually improve your allergy symptoms.

5. In general, I would suggest you to avoid exposure to cold air, dusts, smokes and air pollution as much as possible.

6. For control of symptoms, I would suggest you fexofenadine or ebastine or similar antihistamine (they cause relatively less sedation among other antihistamines) + montelukast.

7. For wheezing, I would suggest you to use salbutamol or levosalbutamol inhaler on as-and-when-needed basis. However, spirometry testing will also help your doctor to choose more appropriate type of inhaler for you.

8. I would like to know your height and body mass index as obesity/overweight could also contribute to your tiredness/sleepiness symptoms. For patients with body mass index more than 25, weight loss as low as 5-10% of the initial body weight can improve symptoms. In that case, I would suggest you to consult a professional dietitian and physical trainer.

9. Regular physical exercise including breathing exercises and a healthy diet rich in vitamins & minerals will also help you in a long run by improving your lung capacity and immunity respectively.
