Hi dear user
i have gone through your questin.
I can understand your concern.

You have a multinoduar goiter, this means that you have thyroid gland that is enlarged and there are some nodules on the structure of the gland.
If you have breathing difficulties a common cause can be the increased goiter and the sympton of nor gaining weight even if you eat a lot can be attributed to hyperthyroidism.

My advice is to consult an endocrinologist and to repeat the thyroid tests TSH,free T3 ,free T4 and an thyroid untrasound to see the actual thyroid fuction and structure.
So your doctor will have the necessary information to deduct if your thyroid gland is increased from the last ultrasound or not and if it the dose of carbimazole is right or not.

The other symptoms like change in taste, thigh pain, nausea and exhaustion in my opinion are not related to your thyroid condition,so i suggest to do a blood test( basic metabolic panel and a complete blood count )to see if you have anemia or any other abnormality that can explain your symptoms. You can talk with your doctor about the tests and the possible conditions that can explain the symptoms.

Hope I have clarified your doubts. If you have any further queries, please feel free to ask. I would be happy to help.
Thank you for using HCM for your health problems.
Best regards.
