What Causes Severe Pain In The Buttock And Leg?

My daughter is having severe pain in right buttox and down back of rt leg. Also feels very bad. We have done Er visits, ct scans, Mri, X-rays, seen bone snd joint, internist, and OBGYN dr. But none of them know what s going on and it s going on 3 weeks now. We are open to suggestions and possibilities please help we do not know where to go from here.

thank you for writing to hcm.i have read history given about your daughter and understand you are very worried.I will try to help in ways i can.

In the history you have not mentioned her age,any history of trauma or nature of work she does,what causes aggravation or relief of the symptoms which could have helped with coming to narrow down the diagnosis.

Coming to the possibilities from the limited information given in query,
there are three possibilities:
1)sciatica which is the compression of the sciatic nerve which might be due to spinal disc protruding and pressing the nerve roots and causing pain.

I would suggest to do a mri spine ,if she hasnt done one already and if done,go for a doppler ultrasound of lowerlimb and ultrasound of the abdomen..

2)piriformis syndrome is another cause of similar pain symptoms and closely related to sciatica.

piriformis is a muscle in the buttock region,which undergoes spasm and causes pain and at times it irritates the nearby sciatic nerve and produces a similar pain.

basically both these are investigated with imaging tests like mri and tests based on moving the leg and see what causes the pain and at what position of the leg and hip

i would recommend rest,see a physical therapist who could suggest exercise and stretching programmes to reduce the sciatic nerve compression.

i would also suggest aceclofenac+thiocolchicoside twice daily for 7 days and pregabalin tablet at night for 10 days and application of heat over the areaa.all this could relieve the pain to a very good extent.

other recommendations would be steroid injection,mild controlled current application etc.

Other infrequent cause is claudication pain due to obstruction in gluteal artery or femoral artery near buttocks which if present will be picked up in doppler scan mentioned earlier.In such cases drugs like cilostazol or low doses of aspirin will help
