What causes severe pain below the ribcage along with vomiting and diarrhea?

My fiancé, Ron, has really bad pain below and under his front left-side ribcage. He said it's a really bad throbbing pain. It's lasted all day, and he has been vomiting all this morning, along with experiencing some diarrhea. This evening, he said it's pretty severe, but he wants to wait until morning to go in to his Doctor. His wanting to wait concerns me. He said when he touches it there, he feels like he needs to vomit. He said it started a couple of days ago, but it seemed to go away. Now this pain has returned, and hurts him very badly and he has to lay down, but cannot sleep. He said it's a sharp, shooting pain. He doesn't feel like he can really sit for any length of time. He has also been burping, and he doesn't usually do this. Do you have any ideas as to what may be possible reasons for his distress? Thank you in advance!! Katrina

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

What the symptoms are given here for abdominal pain, vomiting, seems to be due to infection and stone condition of urinary tract is very likely, condition needs to be confirmed and for that imaging study is must and I would advise for x-ray abdomen plain with Ultrasonography, and to rule out the infection routine urine laboratory examination is advisable, beside this the symptoms of pain and nausea may be due to muscular spasm, instestinal pathology need to be ruled out too.

Last night around this time I got severe pain in my lower abdomen, like menstrual cramping. Then violent vomiting followed by and at the same time diarrhea. It was so bad I slept by the toilet. I have sudden onset severe left upper abdominal pain followed vomiting and a loose stool. It has been 10 hours, my last 2 bowel movements were just mucous and bright red blood. The pain is constant and nothing make it better or worse, occasionally there is cramping. I have drank a little and ate some 5 hours ago, but currently I do not want to eat or drink. I can not sleep because the pain is too bad.
