Hi,

Based on the history you have shared, it can be sign of shingles which may rarely occur with vaccination failure or immunodeficiency states so should be ruled out.

However, if the pain is associated with any focal neurological deficit or fits etc may need brain imaging in that case. The rash must be reviewed by a dermatologist.

Rarely giant cell arteritis or any large or medium-sized vasculitis may present like this so baseline autoimmune profile and inflammatory markers may be needed.

Hope I have answered your query. Let me know if I can assist you further.

Take care
