What Causes Severe Headache After An Injury?

My boyfriend hit his head on Monday. On Tuesday he had a large headache so I gave him ibuprofen for it and told him to go to bed. It is now Wednesday and he had a slight head ache so I gave him a another and sent him for a nap. The headaches are off and on But I just looked at his eyes and his pupils are sightly dilated. Should I take him to the emergency room ? Could he have a concussion or a blood clot ?

Hello dear Greetings
Regarding your experience with your boyfriend headache that wax and wean with slight improvement with ibuprofen following a dilated pupil I will suggest a possible Cerebral hemorrhage following a trauma

In view of this please let him do a CT scan to rule out the above condition and other possible causes of persistent headache such as Cerebral tumor Cerebral thrombosis

Please give him tablet Tylenol pending the outcome of the result
