What Causes Severe Diarrhea With Abdominal Pain?

14 year old boy, 3 days with severe diarrhea, lower abdominal cramping and pain, fever of 103 for 3 days. Returned from Mexico on Friday May 2nd. Rehydrating as much as possible but does not seem to get better. Cannot touch lower abdominal area without wincing in pain. It is general pain and not just located on the right side. Tummy appears to be bloated now with no food eaten other than crackers. At what point do I rule out gastroenteritis lasting too long and when should I consider a trip to the ER?

I have gone through your query and can understand your concerns..

As per your complain the symptoms of severe diarrhea, fever, abdominal cramps along with bloating seems to be due to causes like Food poisoning, Viral Gastroenteritis, parasitic infestation, food allergies etc.
I would suggest you to consult a Gastroenterologist and get evaluated and a thorough clinical evaluation and investigations like Blood tests, stool culture etc can help in diagnosis and treatment can be done accordingly..
You can be advised to take antibiotics if there is bacterial infection while anti parasitic medicines like Albendazole can be given in case of Parasitic infestation..
You can also give Tylenol to reduce fever and pain..
You can continue giving plenty of fluids with oral rehydration salts and also give foods like Yoghurt, rice, bananas, cereals etc..
You can also give Loperaminde to control loose motions..
