What Causes Pus Cells And Blood In Urine?

My father has pus cells in his urine, 5 months back his enlarged prostate was removed and stones were removed from his urinary bladder? Some blood also comes out occasionally with urine. He is consulting a doctor and is on different antibiotics but without much relief.
