Hi,

Noted your history of alternating overactive and under-active thyroid, given Eltroxine, stopped also, stopped smoking and pinching pain started in neck gland, wants to stop smoking but hte pain is starting again.
Well, the condition as per the history you have provided seems to be a condition called Thyroiditis and may be an autoimmune problem.
Hence I would advise you the following:
Consult an Endocrinologist or an ENT surgeon for clinical evaluation, examination and investigations:
MRI for thyroid gland.
Blood tests: CBC with ESR, Free T3, Free T4, TSH to be done on regular basis to decide the need of medicines, Anti-TPO antibodies and as would be advised.
All these will lead to a proper diagnosis and thus have proper management.
Your doses and medicines may need regular adjustment.
Once well settled the pain may not come again in spite of stopping smoking.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, ENT & General Surgeon
