What Causes Painfully Swollen Finger Knuckles?

My hands always seem to be swollen and in constant pain specifically at the knuckles and the first joints in my fingers. Often the pain at night wake me up and most mornings the hands are so swollen I can t make a fist. My medications have not changed.

Looks like a case of Rheumatoid Arthritis if it's an early morning stiffness and swelling. You would need a prolonged treatment with DMARDS and pain killers.
