Urologist 's  Response
Hello
Thanks for query .
Based on the sequence of events and symptoms that your husband is going through I would state that initially he had UTI which got complicated into acute prostatitis which spread retrograde to epididymis causing Epididymitis.

Though he has been taking antibiotics it is likely that the organisms may not be sensitive to those antibiotics making it to get prolonged and complicated . .
He needs to do his urine culture to trace out the organisms causing this infection and antibiotics to which they are sensitive to so that infection can be controlled with appropriate antibiotics,

In the mean while start taking broad spectrum antibiotics like Cefixime along with urinary antiseptic like Nitrofurantoin and anti inflammatory medicine like Diclofenac twice daily and urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Please note Epididymitis and Prostatitis takes long time to get cured hence he will need to take medications for 4-5 weeks .
