What Causes Painful Intercourse?

My fiance were having sex this morning and about five to ten minutes after we started he panicked and had me get off of him because as he put it, I started gushing blood. I had no pain, I just had what I thought was my menstrual cycle two or three weeks ago, I am only 33. I just went in last October because had been bleeding for almost a month. It wasn t heavy so I wasn t worried about losing too much blood. Anyway the doctor said it was normal and prescribed me birth control to stop the bleeding and regulate my cycle again. Now about two or three weeks ago, I had what I assumed to be my menstrual cycle. I was early and only lasted about three or four days. The first day my bleeding was heavier than normal but my cramping wasn t as bad as normal. My cycle is very predictable. I can count out 25 days from the day my cycle ends and I will have it. The even stranger part to me (and I am going into the doctor tomorrow to have an ultrasound and show her the pictures I took, I went into the E.R. today but all they did was a pregnancy test and say she thought I was having my period again) was that my menstrual blood was bright red and normally its a maroon color, and there was a pink/white/Caucasian skin tone tissue mixed in with my normal dark red placenta chunks. Also in this tissue was what looked like to me to be an umbilical cord. It was pink and kind of gray with dark spots inside it. So for me as a person with no medical education, I thought maybe it was a baby. I was out of state so I couldn t go into the doctor to have myself checked out, but I took pictures of it. Anyway as I was saying, this morning during intercourse, I started bleeding. It wasn t any different than normal and I was not in any pain. I went to the bathroom and cleaned myself up and then had to pee. A few minutes later my stomach started to ache, then I started getting a sharp pain that ran the length of my abdomen. From like the top of my tummy (above my belly button) to my vagina muscles and then I started to feel a pressure. To be honest, I have had three children and the it all felt like labor pains. The pressure was the same as the pressure you get when you are in labor. The ache was like contractions right when you start to get them, that dull ache that runs across your whole stomach. Then there was the sharp pain you get that progresses and consumes you and all your thoughts. But it only ran in a stream down my belly now everywhere. Well I decided that the pain might mean an eptopic pregnancy which can have devastating affects. My fiance mentioned at the doctors office today that he has blood on his penis quite often when we have sex, which is something I didn t know until today. So I am not sure what is all going on with me, but I would really like to know why I would start bleeding in the midst of sex? I have read a lot about reasons why this could be happening and most don t pertain to me. It could be cause he is hitting me wrong and causing a scratch of some kind. He says when he is inside me he bottoms out on something and his penis slides to the side. I am not sure if that is my cervix, but if it is, it would explain some pain I have felt in the past and some really good orgasms. Help! I need guidance!!

General Surgeon 's  Response
Hi,

The issue sounds to be vaginal bleed from the highly penetrative or vigorous sex more likely.

Another possibility of vaginal involvement through underlying uterine endometriosis should be sorted out.

Recurrent vaginal infections also present in the same way, and hence vaginal swab would be the first diagnostic procedure to proceed in this regard.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel,
