What Causes Painful Bowel Movement Along With Anal Bleeding?

I'm 39F, just moved to Doha, Qatar from Canada 01-September. Came down with severe strep throat, had to go on antibiotics 24 september (E.R. dose 2 g Rocefin IV push + 5 days Tavanic at home). Strep cleared up, but, you can imagine, drugs wreaked havoc on my GI tract and gave me the worst yeast infection ever - the entire perineal area, like diaper rash. Had diarrhea for 7 days. I've had post-infective IBS and lactose intolerance since 2008 (which seems to have improved), but this Tx did a real number on me.

I got rid of the yeast infection, but when stools returned to normal, had rectal bleeding (bright red) on toilet paper and defecation was slightly painful. Figured it was an anal fissure and tried not to worry. It seemed to improve gradually, almost completely, but then yesterday a.m., excruciating pain during bm - almost in tears, with minor muscle spasms after. Quite a bit of blood on the toilet paper, and it continued to bleed (just a little) till around lunchtime. This morning was slightly less painful, but streak of blood on feces and blood (but less than yesterday) on the toilet paper, but bleeding stopped more quickly. Pain doesn't persist long, but I remain slightly irritated on and off throughout the day. I don't feel any lump anywhere and don't feel like i haven't completely evacuated bowels.

I have a healthy diet, eat lots of F & V, little meat, only lactose-free dairy (for the most part, with some cheating), exercise min. 5 hrs/wk, no smoking, very little alcohol, no family history of colorectal cancer. I admit, the day this re-started I didn't drink nearly enough water. However, I'm scared and unsure of what to do. There are no proctologists here, and I don't relish the idea of going to a male GP for this. I went out and bought benefiber this evening (the only thing they had) and some kind of herbal hemorrhoid cream (everything else they had contained anesthetics, which i didn't think was a good idea. Even their steroidal creams also contained lidocaine or some kind of -caine. I also took a bath (plain water) today. I upped my F & V intake and drank as much H2O as possible today. My question is: given my overall lifestyle, my recent bout of diarrhea, the sudden onset of this problem, lack of any history of constipation, do you think this could be anything other than an anal fissure? Do I really need to see a doctor for this? I read that the vast majority of anal fissures heal on their own. How long does that actually take? Am I doing the right things at home?

Bulk forming laxative as Ispaghula husk 3 teaspoonful at night with a glass of drinking lukewarm water helps by smooth bowel evacuations .
Tab. Ibuprofen ( 400 ) mg after meals for control of pain , burning .
Xylocaine ( 5 % ) jelly application over the anal part gives soothing action .

Sitz bath ( sitting in a tub filled with lukewarm water 5 minutes each 3
times a day and additionally after passing stools ) helps .
Avoid long drives , sitting prolong on hard surfaces .

Control of blood sugar , cholesterol , body weight .
Avoid alcohol consumption .

* Usually simple anal fissures heal within 2 -3 weeks if taken care
precisely , in your case it may be 3 -4 weeks .

* I recommend to follow this for 2 weeks , later if problem persist
visit gastrosurgeon .
